OSHA requires that occupational illnesses and injuries be recorded on the 300 Log. Certain findings must be present before the illness or injury is considered reportable.
Criteria - all of the following:
(1) An illness or injury has occurred.
(2) The condition is work-related or exacerbated by work.
(3) The condition must either:
(3a) meet general criteria for being reportable.
(3b) belong to a special category for reporting.
It is important to determine if the condition is a "new case".
General criteria for being reportable:
(1) being away from work beyond day of occurrence
(2) resulted in job restriction
(3) resulted in job transfer
(4) required medical treatment (beyond first aid)
Special categories:
(1) hearing loss
(2) tuberculosis
(3) sharps/needlestick injury
(4) loss of consciousness
(5) death
(6) other significant injury or illness not receiving medical treatment
