Description

Madoff et al used a combined grading for fecal incontinence and evacuation to evaluate a patient with a disorder of the anal sphincter. This can help monitor a patient with a sphincter disorder or gauge the benefit of sphincter repair. The authors are from the Working Party on Anal Sphincter Replacement with members from across North America and Europe.


 

Parameters:

(1) fecal incontinence

(1a) inconintence for gas or minor staining

(1b) incontinence for liquid or solid stool

(2) difficult stool evacuation

(2a) incomplete or failed evacuation

(2b) need to strain > 30 minutes for stool evacuation

(2c) need to use an enema, irrigation or digital evacuation

• Incontinence for liquid stool is reported as being more distressful for the patient (personal communication from Lowry AC).

 

Interpretation:

• A grade is determined for each type of incontinence and difficult evacuation.

• The final grade is based on the maximum grade in each category.

• The final designation is based on both maximum grades (for example: I-IV, E-2).

 

Observations:

• Introducing a category between "never" and "less than once a month" might be useful. This could be designated "I", while never could be designated "0".
