Description

Ren et al developed a simple score for evaluating a patient with an acute exacerbation of chronic hepatitis B. This can help to identify a patient who may benefit from more aggressive management. The authors are from Zunyi Medical College in China.

Patient selection: acute exacerbation of chronic hepatitis B

Outcome: acute-on-chronic liver failure (ACLF)

Parameters:

(1) age in years

(2) INR

(3) HBV-DNA viral load in copies per mL

R =

= (0.053 * (age)) + (3.631 * (INR)) + (0.553 * (LOG10(viral load))) - 13.323

Interpretation:

• An R value >= -2.43 was associated with ACLF.

Performance:

• The are under the ROC curve was 0.87 for the derivation cohort and 0.83 for the validation cohort.

• The sensitivity was 85% and specificity 80%.
