Description

Kessoku et al reported a score for predicting cirrhosis in a patient with nonalcoholic fatty liver disease (NAFLD). This is a simple score that can help to identify a patient who may need additional studies. The authors are from multiple institutions in Japan and members fo the Japan Study Grop of NAFLD.

PLALA is an acronym for platelet, albumin and AAR.

Patient selection: non-alcoholic fatty liver disease (NAFLD)

Parameters:

(1) platelet count per microliter

(2) serum albumin in g/dL

(3) AST to ALT ratio (AAR)

>= 153,000 per µL

< 153,000 per µL

serum albumin

>= 4.0 g/dL

< 4.0 g/dL

<= 0.9

total score =

= SUM(points for all of the parameters)

Interpretation:

• minimum score: 0

• maximum score: 3

• A score >= 2 is associated with stage 4 liver fibrosis (cirrhosis).

Performance:

• The sensitivity is 87% and specificity 91%.

• The negative predictive value is reported 99.5%

• The positive predictive value is 26%.
