Description

Churchill et al developed the Amphetamine Dependency Questionnaire (SAmDQ) to gauge the severity of a patient's dependency on amphetamine. It is used to evaluate a patient who injects amphetamines. It is modeled on the Severity of Alcohol Dependence Questionnaire (SADQ) and the Severity of Opiate Dependence Questionnaire (SODQ).


 

Sections:

(1) signs of physical withdrawal

(2) signs of affective withdrawal

(3) prevention of withdrawal

(4) recurrence after a period of abstinence

 

Physical signs:

(1) body aches

(2) stomach cramps

(3) feeling sick

(4) heart pounding

(5) hot and cold flashes

 

Affective signs:

(1) depression

(2) tense

(3) irritable

(4) restless

(5) craving

 

Withdrawal prevention:

(1) save some drug for use on waking

(2) drug taken within 2 hours of waking

(3) drug taken to prevent feeling sick

(4) thinking of drugs

(5) drug taken to stop aches

(6) drug taken to stop depression

(7) drug taken to get self going

(8) drug taken first thing after waking
