Functional heartburn can be diagnosed using the Rome IV criteria.
Diagnostic features of functional heartburn - all of the following:
(1) retrosternal burning sensation causing discomfort or pain
(2) no symptom relief while on optimal anti-secretory therapy
(3) frequency of symptoms >= 2 times per week for past 3 months
(4) symptom onset >= 6 months ago
(5) exclusion of gastroesophageal reflux disease (GERD) and eosinophilic esophagitis
(6) exclusion of a major esophageal motor disorder (achalasia, obstruction, diffuse esophageal spasm, jackhammer esophagus, absent peristalsis)
