Total number of weeks with symptoms over the past 12 months

Percent of defecations affected (enter from 0 to 100)

Has the patient been experiencing abdominal pain?

Is there evidence for the irritable bowel syndrome?

Have other causes of chronic diarrhea been excluded after a careful evaluation?

• chronic passage of a large volume of stool?

• rectal bleeding not from hemorrhoids?
