Description

The Children's Hepatic Tumors International Collaboration reported risk stratification for patients with hepatoblastoma. The authors are from multiple institutions from around the world.


Patient selection: hepatoblastoma

 

Parameters:

(1) PRETEXT group (I, II, III, IV)

(2) age in years

(3) VPEFR status at diagnosis (vena cava, portal vein, extrahepatic, multifocal, rupture)

(4) serum AFP in ng/mL

(5) resectability

(6) distant metastases

 

Very low risk:

(1A) PRETEXT I, nonmetastatic, VPEFR negative, resectable at diagnosis

(1B) PRETEXT II, nonmetastatic, < 8 years old, AFP > 100 ng/mL, VPEFR negative, resectable at diagnosis

 

Low risk:

(2A) PRETEXT 1, nonmetastatic, VPEFR negative, not resectable at diagnosis

(2B) PRETEXT II, nonmetastatic, < 8 years old, AFP > 100 ng/mL, VPEFR negative, not resectable at diagnosis

(2C) PRETEXT III, nonmetastatic, < 8 years old, AFP > 1000 ng/mL, VPEFR negative

 

Intermediate risk:

(3A) PRETEXT I, nonmetastatic, VPEFR positive, < 8 years of age

(3B) PRETEXT II, nonmetastatic, < 8 years old, AFP > 100 ng/mL, VPEFR positive

(3C) PRETEXT III, nonmetastatic, < 8 years old, AFP > 1000 ng/mL, VPEFR positive

(3D) PRETEXT III, nonmetastatic, < 8 years old, AFP 101- 1000 ng/mL

(3E) PRETEXT IV, nonmetastatic, < 3 years old, AFP >100 ng/mL

 

High risk:

(4A) metastatic disease

(4B) PRETEXT I, nonmetastatic, VPEFR positive, >= 8 years of age

(4C) PRETEXT II, nonmetastatic, < 8 years old, AFP <= 100 ng/mL

(4D) PRETEXT II, nonmetastatic, >= 8 years old

(4E) PRETEXT III, nonmetastatic, < 8 years old, AFP <= 100 ng/mL

(4F) PRETEXT III, nonmetastatic, >=8 years old

(4G) PRETEXT IV, nonmetastatic, < 3 years old, AFP <=100 ng/mL

(4H) PRETEXT IV, nonmetastatic, >= 3 years old
