Description

Conejo et al reported criteria for risk stratification for a patient with variceal bleeding. This can help identify a patient who may benefit from early TIPS (transjugular intrahepatic portosystemic shunt) placement. The authors are from multiple institutions in Spain, Italy and Canada.

Patient selection: variceal bleeding

Risk Group (6 week mortality)

Child-Pugh Class

MELD

low risk (2-4%)

A

<= 11

intermediate risk (10-12%)

B

12 to 18

high risk (22-46%)

C

>=19

Child-Pugh class C can be divided into 2 groups

(1) serum creatinine < 1 mg/dL (6-week mortality 22%)

(2) serum creatinine >= 1 mg/dL (6-week mortality 52%)
