Description

Nakano et al developed a simple score for identifying a patient with Kawasaki disease who is at high risk for an aneurysm of a coronary artery. A patient at high risk may benefit from more aggressive management. The authors are from Shizuoka Children's Hospital in Japan.

Parameter:

(1) age of the patient in years

(2) platelet count

(3) C-reactive protein concentration (CRP)

Points

age of the patient in years

< 12 months

-1

12 to 24 months

0

> 24 months

+1

platelet count

< 30,000 per µL

-1

>= 30,000 per µL

+1

CRP

< 1.5 mg/dL (0 to 1+)

+2

1.5 to 10.5 mg/dL (2+ to 4+)

+1

10.6 to 14.5 (5+)

0

> 14.5 mg/dL (6+)

-3

where:

• The quantitative values for CRP are based on "best guess" from Table I on page 740.

total score =

= SUM(points for all 3 parameters)

Interpretation:

• minimum score: -5

• maximum score: +4

• A score < 0 indicates high risk for coronary artery aneurysm.

• A score >= 0 indicates a low risk for coronary artery aneurysm although some cases occurred at scores of 0 ot +1.
