Description

DeCarlo et al reported a score for predicting the risk of the abdominal compartment syndrome after endovascular repair of a ruptured abdominal aortic aneurysm. This can help to identify a patient who may require more aggressive management.  The authors are from Massachusetts General Hospital and University of Massachusetts.

Patient selection: endovascular repair of a ruptured aortic aneurysm

Parameters:

(1) transfer from outside hospital

(2) serum creatinine in mg/dL

(3) systolic blood pressure in mm Hg

(4) mental status

(5) liters of intraoperative crystalloid

(6) units of PRBCs transfused intraoperatively

Interpretation:

• minimum score: -1

• maximum score: 13

• A score >=8 has a sensitivity of 0.88 and specificity of 0.83 for the abdominal compartment syndrome.

Performance:

• The area under the ROC curve is 0.84 in the derivation and 0.87 in the validation cohorts.
