Description

Catelvecchio et al developed a score for predicting morality during surgical ventricular reconstruction (SVR) for heart failure. This can help to identify a patient who may benefit from an alternative or more aggressive therapy. The authors are from IRCCS Policlinico San Donato in Milan.

Patient selection: SVR for heart failure

Parameters for ACEF score (of Ranucci et al)

(1) age (A)

(2) serum creatinine (C)

(3) left ventricular ejection fraction (EF) in percent (as a number from 0 to 100)

Serum Creatinine

Points

< 2 mg/dL

>= 2 mg/dL

ACEF score =

= ((age) / (ejection fraction)) + (points for serum creatinine)

Additional parameters:

(1) atrial fibrillation

(2) NYHA class 3 or 4

(3) mitral valve surgery

ACEF-SVR score =

= (ACEF score) + SUM(points for 3 parameters)

X =

= (0.83 * (ACEF-SVR score)) - 5.4

probability of operative mortality =

= 1 / (1 + EXP((-1) * X))
