Description

Fang and Xu used the ratio of serum BUN to albumin to predict short-term mortality for a patient with an acute pulmonary embolism. This can help to identify a patient who may benefit from moer aggressive management. The authors are from Ningbo Medical Center Lihuili Hospital and University of Chinese Academy of Sciences in Zhejiang Province in China.


Patient selection: acute pulmonary embolism

 

Parameters:

(1) serum BUN in mg/dL

(2) serum albumin in g/dL

 

BUN to albumin ratio in mg per g =

= (serum BUN) / (serum albumin)

 

Interpretation:

• A ratio >= 7.979 is associated with a worse 28-day mortality rate with a hazards ratio of 1.07.

 

Performance:

• The area under the ROC curve is 0.80.

 

Other factors predictive of mortality from multivariate Cox regression analysis:

(1) low SpO2

(2) malignancy

(3) leukocytosis

(4) hyperglycemia

(5) need for vasopressors

(6) elevated SAPSII
