Description

van den Akker et al reported findings associated with acute kidney injury (AKI) in a patient with cardiogenic shock. These can help to identify a patient who may require more aggressive management. The authors are from Erasmus University, Columbia University, New York University, and Pontificia Universidad Catolica de Chile.


Patient selection: cardiogenic shock

 

Findings associated with acute kidney injury:

(1) high central venous pressure (CVP; mean 15.5 with AKI vs 12 without)

(2) low diastolic arterial blood pressure in mm Hg (mean 52 with AKI vs 66 without)

(3) low mean perfusion pressure in mm Hg (mean 53 with AKI vs 68 without)

(4) higher maximum ventilator pressure and PEEP

(5) higher doses of vasopressor support

Only CVP was significant in multivariate analysis.
