Description

Zaoutis et al identified risk factors for candidemia in a pediatric patient in the pediatric intensive care unit. These can help to identify patiets who should be monitored more closely and treated aggressively. The authors are from the Children's Hospital of Philadelphia, University of Pennsylvania and National Cancer Institute.


 

Mortality in children with candidemia was 44% vs 14% in controls.

 

Major risk factors (odds ratio 30):

(1) central venous catheter

 

Intermediate risk factors (odds ratios 3.5 to 6.2):

(1) presence of a malignancy

(2) use of vancomycin for 4 or more days during the past 2 weeks

(3) use of antibiotics active against anaerobic bacteria for 4 or more days during the past 2 weeks

 

Additional risk factors (odds ratio 2 to 3)

(1) use of vancomycin for 1 to 3 days during the past 3 weeks

(2) receipt of total parenteral nutrition

 

Patients with a > 10% risk for candidemia:

(1) central venous catheter + malgianncy + vancomycin 4+ days (11% risk)

(2) central venous catheter + 3 or 4 of the other risk factors (12 to 46%) with the highest risk seen when all of the risk factors are present
