Description

Wan et al reported risk factors associated with 90-day readmission for a geriatric rehabilitation patient. These can help to identify a patient who may benefit from interventions to reduce readmission. The authors are from the University of Melbourne, Vrije Universiteit Amsterdam, and the National University of Singapore.


Patient selection: mean age 82.2 +/- 7.8; nonplanned readmission

 

90-day readmission rate: 25%

 

Risk factors for 90-day readmission from multivariate analysis:

(1) high fear of falling

(2) high Cumulative Illness Rating Scale (CIRS)

 

Additional risk factors:

(1) no tertiary education

(2) poor self-reported quality of life

(3) high Charlson comorbidity index (CCI)

(4) polypharmacy

(5) receipt of formal care (skilled nursing, nursing home, etc) after discharge

(6) immigrant
