Vestergaard et al reported risk factors for inflammatory bowel disease (IBD) activity during pregnancy. These can help to identify a woman who may benefit from more aggressive management. The authors are from Aarhus University, Aalborg University and Brigham and Women's Hospital.
Patient selection: pregnant woman with a history of IBD
Risk factors for disease activity:
(1) ulcerative colitis (aOR 2.6)
(2) active disease <= 6 months prior to conceiving (aOR 5.3; increased risk continuous disease activity)
(3) history of disease activity in previous pregnancy (aOR 3.2; increased risk of flares)
If there is a sustained remission throughout the pregnancy, then there is:
(1) a reduced risk of stillbirth
(2) a reduced risk of fetal growth restriction
(3) an increased chances for normal term delivery and birthweight
