Description

Valencia et al evaluated patients with severe community-acquired pneumonia as identified by the Pneumonia Severity Index (PSI, above). The authors are from the University of Barcelona and Winthrop-University Hospital (Mineola, New York).


 

Factors associated with being admitted to the ICU (by multivariate analysis):

(1) age <= 80 years

(2) cirrhosis

(3) diabetes mellitus

(4) renal impairment (serum creatinine > 1.6 mg/dL)

(5) PaO2 to FIO2 ratio < 250

(6) diastolic blood pressure < 60 mm Hg

(7) acidosis (pH < 7.35)

 

Factors associated with mortality (by multivariate analysis):

(1) aspiration (odds ratio 2)

(2) low level of consciousness or coma (odds ratio 3)

(3) shock (odds ratio 23)
