Description

Trivedi et al identified risk factors for hepatocellular carcinoma arising in a patient with PBC (primary biliary cirrhosis or cholangitis). These can help to identify a patient who may benefit from closer monitoring. The authors are from multiple institutions participating in the Global PBC Study Group.


Patient selection: PBC

 

Outcome: development of hepatocellular carcinoma

 

Risk factors for development of hepatocellular carcinoma:

(1) male gender

(2) advanced disease at initial presentation (cirrhosis, hepatic decompensation, abnormal serum total bilirubin and albumin)

(3) failure to respond to ursodeoxycholic acid at 12 months(Paris I criteria)

 

Additional factors:

(4) thrombocytopenia (probably reflects hypersplenism in more advanced disease)

(5) older age at diagnosis
