Description

Trencheva et al identified risk factors for an anastomotic leak after a colorectal resection. These can help to identify a patient who may benefit from more aggressive management. The authors are from Weill Cornell Medical College, Cornell University and the Hospital for Special Surgery in New York City.


Patient selection: colorectal resection

 

Outcome: anastomotic leak

 

Preoperative and intraoperative risk factors for anastomotic leak:

(1) male sex (odds ratio 2.3)

(2) Charlson comorbidity index >= 3 (odds ratio 3.1)

(3) ligation of the inferior mesenteric artery above the left colic artery ("high" ligation, odds ratio 3.8)

(4) intraoperative complications (odds ratio 4.1)

(5) anastomosis less than 10 cm from the anal verge (odds ratio 4.1)

 

A patient with multiple risk factors would be at increased risk for anastomotic leak.
