Description

Taran et al reported a meta-analysis for predictors of extubation failure in patients with acute traumatic brain injury (TBI). These can help to identify a patient who may require greater care during extubation. The authors are from multiple institutions in Canada, Thailand and Europe.


Patient selection: traumatic brain injury (TBI) undergoing extubation

 

Risk factors for extubation failure with moderate certainty:

(1) older age (OR 3.0)

(2) longer duration of mechanical ventilation (OR 3.5)

 

Factors associated with low risk of extubation failure:

(1) cough (OR 0.4)

(2) swallow (OR 0.34)

 

Additional factors with low certainty:

(1) GCS 3 or 4 (OR 2.7)

(2) higher PaO2/FIO2 ratio (OR 0.26)
