Description

Sun et al identified risk factors for a infection associated with extracorporeal membrane oxygenation (ECMO) therapy in an adult. These can help to identify a patient who may benefit from more aggressive management. The authors are from National Taiwan University in Taipei and the University of Pittsburgh.


Patient selection: adult undergoing ECMO therapy

 

Outcome: infection

 

Sites of infection include:

(1) bloodstream

(2) pneumonia

(3) surgical site

(4) urinary tract

 

Risk factors for infection:

(1) increased duration of ECMO therapy (odds ratio 1.003): duration in patients with infection from 123 to 597 hours (mean 360); duration in patients without infection associated with use 0 to 320 hours (mean 157); will use > 300 hours in the implementation

(2) mechanical complications (odds ratio 4.8): oxygenator failure, pump malfunction, heater dysfunction, clotting, connector crack, etc

(3) autoimmune disease (odds ratio 7.0): SLE, Wegener's, dermatomyositis, etc

(4) venovenous mode (odds ratio 4.5)

 

The risk of infection increases with the number of risk factors present.
