Description

Singh et al identified a number of factors associated with a complicated course following esophageal perforation. These can help to identify a patient who may require more aggressive management. The authors are from the Sanjay Gandhi Post Graduate Institute of Medical Sciences in Lucknow.


 

Patient selection: esophageal perforation

 

Risk factors for morbidity and mortality:

(1) late presentation (more than 2 days after the event)

(2) low serum albumin (less than 3 g/dL)

(3) perioperative mechanical ventilation (respiratory failure)

 

No one in the author’s series died if the presentation was less than 2 days.

 

Other factors to consider:

(1) delay in referral or proper management

(2) serious comorbid conditions (which may limit options)
