Shihota et al reported risk factors for visual field defects for a patient with primary congenital glaucoma. This can help to identify a patient who may benefit from more aggressive management. The authors are from All India Institute of Medical Sciences in New Delhi.
Patient selection: primary congenital glaucoma
On multivariate analysis the single most important risk factor was female sex.
Other factors from univariate analysis:
(1) need for bleb revision (OR 9.8)
(2) repeat trabeculectomy with mitomycin-C (OR 18)
(3) final cup-to-disc ratio (CDR) > 0.8 (OR 23.1)
The need for >= 2 medications was also identified as a possible risk factor.
Baseline CDR
Grade of Cupping for Later Visual Field Loss
< 0.6
mild
0.6 to 0.8
moderate
> 0.8
severe
