Description

Sharma et al identified risk factors for a surgical site infection (SSI) at the site where saphenous vein grafts were harvested prior to a coronary artery bypass procedure. A patient with risk factors for surgical site infection may benefit from more aggressive management. The authors are from St. John Hospital and Medical Center and Wayne State University in Detroit.

Patient selection: coronary artery bypass graft (CABG)

About two thirds of the SSI were superficial while the remaining third were deep.

Risk factors for a harvest surgical site infection:

(1) older age (mean age for SSI 70 vs 67 for non-SSI)

(2) renal insufficiency

(3) diabetes mellitus

(4) body mass index (BMI) > 30 kg per square meters

(5) congestive heart failure

(6) prolonged duration of the surgery

(7) female gender

Common pathogens included:

(1) Staphylococcus aureus

(2) Pseudomonas aeruginosa

(3) enteric bacteria
