Description

Saxena et al identified risk factors for massive blood transfusion associated with cytoreductive surgery. These can help to identify a patient who may require more aggressive management. The authors are from the University of New South Wales in Sydney, Australia.


Patient selection: peritoneal carcinomatosis undergoing cytoreductive surgery with intraperitoneal chemotherapy

 

Outcome: massive allogeneic red blood cell transfusion

 

Risk factors for massive transfusion:

(1) operative length > 9 hours

(2) preoperative hemoglobin < 125 g/L

(3) peritoneal cancer index >= 16

(4) preoperative INR >= 1.2

(5) number of peritonectomy procedures >= 4

 

where:

• Surgery prior to 2004 was also associated with risk of massive transfusion.

 

Several of the factors reflect high tumor burden.
