Description

Saber et al identified risk factors for an intravascular-related thrombosis in a patient with cancer. These can help to identify a patient at risk who may benefit from anticoagulation. The authors are from multiple hospitals in North America and Europe.


 

Patient selection: cancer patient with intravascular catheter

 

Risk factors for thrombosis:

(1) peripherally implanted central venous catheter (PICC) line (versus implanted port)

(2) past history of deep vein thrombosis (DVT)

(3) subclavian venipuncture insertion technique

(4) catheter tip not in right atrium or at junction of superior vena cava and right atrium

 

where:

• A history of catheter-related thrombosis might be an additional risk factor.

• A history of thrombophhilia might be added to the history of DVT.

• Missing data seemed significant for several of the variables.

• Subclavian venipuncture and PICC line are mutually exclusive.

 

Factors not reaching statistical significance:

(1) triple lumen catheter (odds ratio 3.8, p value 0.10)

(2) technical difficulties (odds ratio 1.8,p value 0.16)
