Description

Rello et al identified risk factors for Pseudomonas aeruginosa-associated ventilator associated pneumonia (VAP). These can help to identify patients who may benefit from more specific therapy. The authors are from Joan XXIII University Hospital in Tarragona, Spain.


 

Patient selection: ICU patient with ventilator-associated pneumonia following recent antibiotic exposure

 

Risk factors for Pseudomonas aeruginosa as the etiologic agent on multivariate analysis:

(1) onset 5 days or more after admission to the ICU (> 4 days)

(2) adult respiratory distress syndrome (ARDS)

(3) absence of coma

(4) corticosteroid use

 

Additional factors seen on the univariate analysis:

(1) dialysis

(2) COPD

(3) nonsurgical patient

 

VAP with onset during the first 4 days of ICU admission is usually due to another organism.
