Description

Reincke et al identified factors in a patient with primary aldosteronism that are associated with a decrease in glomerular filtration rate (GFR). These can help to identify a patient who may benefit from interventions to maintain renal function. The authors are from multiple universities in Germany.


 

Patient selection: primary aldosteronism

 

Outcome: decline in glomerular filtration rate (GFR)

 

Risk factors associated with a decline in renal function in an untreated patient:

(1) advancing age (there is a progressive decline with age, will use >= 60 years in the implementation)

(2) male gender

(3) low serum potassium (marker of disease activity)

(4) high serum aldosterone concentration (marker of disease activity)

 

Therapeutic interventions associated with a decline in renal function

(1) adrenalectomy

(2) therapy with spironolactone
