Plath et al identified risk factors for dislocation arthropathy following arthroscopic Bankart repair. These can help to identify a patient who may benefit from more aggressive management. The authors are from Technische Universitat Munchen.
Patient selection: arthroscopic Bankart repair of shoulder dislocation
Outcome: radiographic evidence of glenohumeral osteoarthritis
Glenohumeral osteoarthritis was present in about 70% of patients during follow-up, with half of these cases mild. About 10% of all patients developed severe osteoarthritis.
Risk factors for glenohumeral osteoathritis:
(1) greater number of preoperative dislocations (use >= 5 in the implementation)
(2) age at the initial dislocation (use >= 24 years in the implementation)
(3) age at the initial surgery (use >= 29 years in the implementation)
(4) number of anchors (fixation devices) used during surgery (use >= 4 in the implementation)
Recurrent dislocation after operative repair was not associated with development of osteoarthritis in the study group but this might need to be reassessed after a longer follow-up period.
