Description

Phair et al identified risk factors for problems arising after a major amputation of a lower extremity. These can help to identify a patient who may benefit from more aggressive management. The authors are from Montefiore Medical Center and Massachusetts General Hospital.

Patient selection: major amputation of a lower extremity (above-the-knee, below-the-knee)

Outcome: 30-day complications (overall 30-day readmission rate was 29%)

Risk factors for 30-day unplanned readmission:

(1) end-stage renal disease

(2) ASA 4 (ASA 3 protective)

(3) coronary artery disease

Risk factors for 30-day stump complications:

(1) below-the-knee amputation

(2) rest pain

Risk factors for 30-day stump revision:

(1) below-the-knee amputation

(2) rest pain

(3) occluded bypass graft
