Perbet et al identified risk factors for cardiovasacular collapse following emergency endotracheal intubation of a patient in the intensive care unit (ICU). The authors are from University Hospital of Clermont-Ferrand in France.
Patient selection: emergency endotracheal intubation in the ICU in patients who were previously hemodynamically stable without the need for vasoactive drugs
Outcome: severe cardiovascular collapse
Risk factors for severe cardiovascular collapse:
(1) age >= 60 years of age (risk greater if age > 75 years)
(2) noninvasive ventilation prior to intubation
(3) acute respiratory failure as indication for intubation
(4) FIO2 > 70% after intubation
(5) not comatose (odds ratio if comatose 0.48)
(6) higher SAPS II score
(7) initial intubation being performed in the ICU
where:
• SAPS II score if no collapse 35.8 +/- 16.5 vs with collapse 39.3 +/- 17.1. There is considerable overlap. A SAPS II score > 42 will be arbitrarily used in the implementation.
