Description

Park et al identified risk factors for recurrence or death following complete resection of a Stage I gastric cancer. These can help to identify a patient who may benefit from more aggressive management. The authors are from Asan Medical Center and University of Ulsan in South Korea.

Patient selection: status post complete resection of a Stage I gastric cancer

5-year recurrence free survival: 90%

5-year overall survival: 93%

Risk factors for recurrence or death:

(1) age > 65 years (HR 4.6)

(2) male gender (HR 1.6)

(3) stage IB (HR 2.1)

(4) lymphovascular invasion (HR 1.7)

(5) perineural invasion (HR 1.9)

(6) elevated serum CEA concentrations (above normal; HR 1.8)

The presence of 3 or more risk factors is at significant risk of recurrence or death. These patients may benefit from adjuvant chemotherapy.
