Description

Park et al identified a number of risk factors associated with an anastomotic leak following a laparoscopic resection of a rectal carcinoma. These can help to identify a patient who may benefit from a diverting colostomy. The authors are from Kyungpook National University Medical Center in Daegu with the data collected from 11 institutions in Korea.


 

Patient selection: laparoscopic resection of a rectal carcinoma

 

Risk factors for an anastomotic leak:

(1) male gender (HR 3.5)

(2) advanced tumor stage (HR 2.5)

(3) tumor less then 7 cm from the anal verge (HR 2.4)

(4) preoperative neoadjuvant chemoradiation (HR 6.2)

(5) perioperative transfusion (perioperative bleeding) (HR 10.7)

(6) multiple firings of the linear stapler (HR 6.2)

 

If 2 or more of these risk factors are present then the patient may be a candidate for a diverting colostomy.
