Description

Panzuto et al identified factors associated with disease progression in patients with a pancreatic endocrine carcinoma. These can help to identify patients who may benefit from more aggressive management. The authors are from multiple universities and cancer centers in Italy.


 

Patient selection:

(1) pancreatic endocrine tumor that was locally advanced or metastatic

(2) advanced disease not suitable for radical surgery, or residual disease after debulking surgery

(3) absence of a familial MEN syndrome

 

Risk factors for disease progression:

(1) poorly differentiated endocrine carcinoma (Grade 3)

(2) Ki-67 index > 5%

(3) no treatment after diagnosis

 

where:

• Ki-67 index is the percent of 2,000 cells in the area of maximal staining that are positive for the immunostain to Ki-67 (MIB1).

• Grade 1 tumors were associated with a Ki-67 index <= 5%.

 

Metastases when present were to most often located in:

(1) liver

(2) lung

(3) bone
