Description

Pagano et al identified risk factors for invasive fungal infection (IFI) in a pediatric patient with a hematologic malignancy. These can help to identify a patient who may benefit from more aggressive management. The authors are members of SEIFEM (Sorveglianza Epidemiologica Infezioni Fungine nelle Emopatie Maligne) Group in Italy.


Risk factors based on hematologic malignancy:

(1) type of hematopoietic malignancy (AML, high risk ALL, relapsed ALL)

(2) admission to PICU

(3) central venous catheter

(4) severe and prolonged neutropenia

(5) persistent fever > 4 days with monocytopenia and elevated CRP

 

Risk factors associated with an allogeneic stem cell transplant:

(1) GVHD

(2) high-dose corticosteroid therapy >= 2 mg/kg [e day

(3) risk of treatment related mortality > 20%

(4) adolescent
