Oliver et al evaluated adults with congenital heart disease for risk factors associated with increased mortality. The authors are from multiple hospitals in Spain.
Patient selection: adult with congenital heart disease
Outcome: excess mortality
Parameters:
(1) complexity of congenital heart disease
(2) reparability
(3) features
(4) complications
Complexity and repairability - the mortality is increased:
(1) for severe complexity >> moderate > simple.
(2) if the heart disease is not reparable.
Features associated with increased mortality:
(1) genetic syndrome
(2) severe pulmonary outflow tract obstruction
(3) left and/or right venticular dysfunction
(4) single ventricle physiology
(5) clinical cyanosis (indicative of incomplete repair)
(6) significant subaortic atrioventricular valve regurgitation
Complications associated with increased mortality:
(1) pulmonary hypertension
(2) aortic aneurysm
(3) infective endocarditis
(4) ischemic heart disease
