Description

Oliveira et al reviewed reports of surgical site infections in liver transplant recipients. These can help to identify a patient who may benefit from more aggressive management. The authors are from Sao Paula University.


Patient selection: liver transplant recipient

 

Risk factors:

(1) MELD score > 35

(2) preoperative mechanical ventilation on day of surgery

(3) net ascites fluid output > 1 liter during surgery

(4) difference in age between donor and recipient > 10 years

(5) difference in sex between donor and recipient

(6) ratio of donor liver mass to recipient body mass < 0.01

(7) annual number of liver transplants at the transplant center less than 50

(8) transfusion of packed red blood cells

(9) extended surgical time

(10) hyperglycemia > 200 mg/dL during surgery

(11) use of vasopressors during surgery

 

where:

A liver to body weight ratio of 0.01 in a 75 kg patient indicates a liver weighing 750 grams.
