Description

Naqvi et al evaluated patients with paravalvular abscess for risk factors associated with 1 year mortality. These can help identify patients who may benefit from more aggressive management. The authors are from Cedars Sinai Medical Center and UCLA School of Medicine in Los Angeles.


 

Patient selection: infective endocarditis with paravalvular abscess

 

Risk factors for mortality during hospitalization and at 1 year:

(1) history of renal dysfunction, including chronic hemodialysis

(2) altered mental status (confusion, etc) or stroke on presentation

(3) diabetes mellitus at presentation

(4) hypertension (which may reflect renal disease)

 

The more risk factors that are present, the greater the mortality at 1 year.

 

Surgical therapy is preferable to medical management if the patient can tolerate the procedure. Patients who did not undergo surgery tended to have serious underlying diseases.
