Description

Montravers et al reported risk factors associated with treatment failure or death for a patient with postoperative peritonitis. The authors are from multiple institutions in France.


Patient selection: post-operative peritonitis

 

Risk factors for overall failure:

(1) documented piperacillin/tazobactam therapy (aOR 2.1)

(2) renal replacement therapy (RRT) on the day of reoperation (aOR 3.0)

 

Risk factors for death (any cause):

(1) older age (aOR 1.08; range for survivors 57 to 74; range for nonsurvivors 64 to 81)

(2) renal replacement therapy (RRT) on the day of reoperation (aOR 4.0)

(3) diabetes (aOR 7.0)

 

Risk factors for surgical failure (need for reoperation or additional abdominal drainage):

(1) documented piperacillin/tazobactam therapy (aOR 2.0)

(2) positive peritoneal culture for Klebsiella species (aOR 2.5)

(3) pancreas as source of infection (aOR 2.9)

 

No risk factors for found for microbiological failure (another course of antibiotic therapy >= 48 years after stopping antibiotics in order treat recurrent infection; superinfection between day 8 and 45).
