Description

Metz et al identified risk factors associated with delayed bleeding following endoscopic resection of a laterally-spreading colonic tumor. The authors are from Westmead Hospital in Sydney, Australia.


Patient selection: endoscopic resection of a large laterally spreading tumor in the colon

 

Outcome: delayed bleeding that is significant (requires hospital admission)

 

Frequency: 7%

 

Risk factors for delayed bleeding:

(1) location in right colon (odds ratio 4.4)

(2) use of aspirin (odds ratio 6.3)

(3) older age (odds ratio 1.7 per decade)

 

Bleeding occurred before aspirin was restarted. Aspirin's effect on platelet function can last for over 7 days after the last dose).

 

Factors not found to be predictive:

(1) comorbidity

(2) ASA grade

(3) size of lesion

(4) number of excisions

(5) polyp morphology

(6) previous attempts,
