Description

Mancone et al compared risk factors for surgical site infection (SSI) following coronary artery bypass graft (CABG) versus percutaneous coronary intervention (PCI) for multivessel coronary artery disease. Surgical site infections following a coronary artery procedure are associated with increased morbidity and mortality. The authors are from multiple institutions in Europe and the United States participating in a SYNTAX trial.

Patient selection: multivessel coronary disease

Outcome: major infection during 5-year follow-up

The risk of infection:

(1) was greater for CABG (16% vs 5% for PCI)

(2) mostly occurred within the first 60 days after the procedure

Risk factors following CABG surgery:

(1) body mass index (BMI; 1.05 per kg per sq meter increase)

Risk factors following PCI:

(1) older age (odds ratio 1.6 per 10 year increase)

(2) peripheral vascular disease (odds ratio 2.8)

(3) subsequent need for CABG (odds ratio 21.4)
