Description

Lo Vecchio et al identified risk factors for severe outcomes in children with measles. These can help to identify a patient who may require more aggressive management. The authors are from multiple institutions in Italy participating in the SITIP Measles Study Group.

Patient selection: measles

Outcome: severe outcome (ICU admission, death, discharged with permanent sequelae), which occurred in about 7% of patients

Risk factors for severe outcome in multivariate analysis:

(1) encephalitis

(2) pancreatitis

(3) serum CRP concentration > 2 mg/dL

Other factors that may impact outcome:

(1) diarrhea (p value 0.06 in multivariate analysis)

Complications that can occur:

(1) pneumonia

(2) oitits

(3) laryngitis

(4) encephalitis

(5) myocarditis

(6) pancreatitis

(7) myocarditis

(8) diarrhea

(9) cytopenias (neutropenia, lymphopenia, thrombocytopenia)

(10) thrombophlebitis

(11) multi-organ failure

The B3 genotype was associated with neutropenia.
