Rineer et al reported risk factors for bacteremia in children and young adults with sickle cell disease (SCD). These can help to identify a febrile patient who may require more aggressive management. The authors are from Cincinnati Children's Hospital, Children's Hospital of Wisconsin, and Medical College of Wisconsin.
Patient selection: febrile and < 22 years of age
Frequency of bacteremia diagnosed within 3 days of index visit: 1.1%
Risk factors from multivariate analysis:
(1) history of CLABSI (OR 6.4)
(2) history of bacteremia (OR 1.4)
(3) history of apheresis (OR 1.8)
Factors associated with lower risk:
(1) hemoglobin SC (OR 0.76)
Additional risk factors from univariate analysis:
(1) history of osteomyelitis
(2) history of stroke
