Description

Liang et al identified risk factors for surgical site infections in geriatric patients undergoing orthopedic surgery. These can help to identify a patient who may benefit from more aggressive management. The authors are from The First Hospital of Jilin University and Xinjiang Medical University in China.


Patient selection: age >= 60 years of age with orthopedic surgery

 

Outcome: superficial or deep surgical site infection (SSI)

 

Staphylococcus aureus caused half of the infections, with half of these MRSA.

 

Risk factors for SSI:

(1) diabetes mellitus (OR 3.7)

(2) morbid obesity (OR 2.6)

(3) tobacco use (OR 4.2)

(4) surgical duration > 75th percentile (odds ration 1.9; roughly 1 SD above mean)

(5) serum albumin concentration < 3.5 g/dL (odds ratio 2.3)

 

The risk of SSI increases with the number of risk factors present. Several of the risk factors may be amenable to intervention prior to surgery.
