Li et al identified factors affecting bile leakage after hepatectomy for hepatolithiasis. These can help to identify a patient who may benefit from a change in or more aggressive management. The authors are from Sun Yet-san University in Guangzhou, China.


Patient selection: hepatectomy for hepatolithiasis

 

Outcome: bile leak

 

Risk factors for bile leakage on multivariate analysis:

(1) recent episode of acute cholangitis (less than 1 month)

(2) left hepatectomy

 

Factors associated with reduced risk of bile leakage:

(1) preoperative cholangiography to identify an aberrant hepatic duct

 

Patients with bile leakage were at increased risk for:

(1) sepsis

(2) subphrenic abscess

(3) surgical site infection

(4) gastrointestinal bleeding
