Description

Lee et al identified risk factors for postoperative pneumonia in patients undergoing surgery for lung cancer. These can help to identify a patient who may benefit from more aggressive management. The authors are from Seoul National University.

Patient selection: adult with lung cancer following surgery

Outcome: post-operative pneumonia (incidence 6.2%, 27% in-hospital mortality)

Independent risk factors for postoperative pneumonia from multivariate analysis:

(1) age >= 70 years (odds ratio 3.6)

(2) intraoperative RBC transfusion (odds ratio 4.7)

(3) postoperative complications other than pneumonia (odds ratio 3.0)

(4) FEV1 to FVC ratio < 70% (odds ratio 3.9), with FEV1 and FVC in liters

The risk of postoperative pneumonia increases with the number of risk factors present.
