Description

Lancellotti et al identified risk factors for systemic embolization and death in a patient with infective endocarditis. These can help identify high risk patients who may require more aggressive management. The authors are from the University of Liege in Belgium and University Hospital of Bari, Italy.

Risk factors associated with increased mortality:

(1) systemic embolization

(2) echocardiographic evidence of a paravalvular abscess

(3) Staphylococcus aureus as the causative organism

(4) medical treatment only (no surgical therapy)

Risk factors for systemic embolization:

(1) total length of vegetations > 14 mm

(2) greater age (affected group had an age 61 +/- 3 years vs 51 +/- 4 for unaffected; an age >= 58 years will be used in the implementation)

(3) medical treatment only

total length of all vegetations =

= SUM(height of all vegetations present)
