Description

Keren et al reported risk factors for recurrent febrile/symptomatic urinary tract infection (UTI) in a child. These can help to identify a child who may benefit from more aggressive management. The authors are from Children's Hospital of Philadelphia, University of Pennsylvania, University of Pittsburgh, Children's Hospital of Pittsburgh, Children's National Health System (Washington, DC) and University of North Carolina.


Patient selection: child with febrile or symptomatic urinary tract infection

 

Risk factors for recurrent febrile or symptomatic urinary tract infections:

(1) Grade III or IV vesicoureteral reflux (hazard ratio 1.9)

(2) bladder and bowel dysfunction (hazard ratio 2.1)

(3) baseline renal scarring (hazard ratio 2.9)

 

where:

Bladder and bowel dysfunction is measured using a modified dysfunctional voiding questionnaire.

 

The risk of recurrent UTI increases with the number of risk factors present.
