Description

Jabbour et al identified predictors for mortality in patients with necrotizing fasciitis in Qatar. These can help to identify a patient who may benefit from more aggerssive management. The authors are from Hamad General Hospital and Weill Cornell Medical School in Doha, Qatar.


Patient selection: adult with necrotizing fasciitis (27% perineum; 52% lower extremity)

 

Most patients were male (74%).

 

Risk factors for mortality in multivariate analysis:

(1) older age (61.6 +/- 14.3 vs 47 +/- 14; use > 60 years in model)

(2) higher SOFA score (range from 0 to 24; mean 12 vs 9)

 

Non-survivors tended to have:

(1) diabetes

(2) hypertension

(3) coronary artery disease

(4) less pain

(5) less fever

(6) a high frequency of septic shock

(7) a high serum creatinine
